Document and Report Incidents involving LAUSD/UTLA

These steps will help you clearly record what happened and create a report that can be shared with the school, legal representatives, or advocacy organizations. Your involvement is crucial, and we’re here to support you.

Report an incident

1. Act Quickly

As soon as you are aware of an incident, jot down the key details while they're fresh in your mind. It doesn't need to be perfect—just capture the essentials.

2. Keep it Simple

When writing down what happened, focus on the who, what, when, and where of the incident:

Who was involved?
What happened?
When did it happen (date and time)?
Where did it happen (classroom, playground, online, etc.)?

3. Stay Factual

Stick to the facts as much as possible. Write what you saw or heard, not how you feel about it. If your child told you what happened, use their exact words where possible.

4. Take Photos or Screenshots (if applicable)

If there are physical signs (like damage to property) or digital evidence (like a message), take clear photos or screenshots and note when you took them.

5. Record Names and Titles

Make sure to note the names and titles of any staff involved, including teachers, counselors, or administrators. If others witnessed the incident, write down their names as well.

6. Be Consistent

Keep all records in one place—whether a notebook or digital document. Add updates if there are follow-ups or new incidents, and make sure to date each entry.

7. Compile Your Report

When you're ready to create a formal report, combine your notes into a single document. Include all relevant details and any evidence you’ve gathered, like photos or screenshots. A straightforward timeline is often the easiest way to present everything.

8. Stay Calm and Organized

We know this can be an emotional process, but taking a calm, organized approach helps ensure your concerns are taken seriously. You are doing the right thing by advocating for your child.
